The wholesale registration form

Publish a branded signup form so buyers apply from your storefront and approval creates the customer.

Instead of taking wholesale applications over email, publish a real signup form on your storefront. Applications arrive in an inbox inside the app, and approving one creates the Shopify customer, applies the group tag, and switches their pricing on immediately.

Turning it on

  1. Go to Registration Form in the app.
  2. Set the heading, description, and which group approved applicants are tagged into.
  3. Toggle Enable form on.
  4. In your theme editor, add the Wholesale Registration Form block to any page, or link buyers to the hosted form URL shown in the app.
The form is off by default. Nothing appears on your storefront until you enable it, so installing the app never changes what your customers see.

It matches your theme automatically

The form reads the fonts, colors, and button styling from your live theme, so it looks like a page you built rather than an embedded app widget. There is no CSS to write. If you change your theme, the form follows.

Custom fields

Custom fields are available on Growth and Pro plans. Every plan includes the standard fields (business name, contact name, email, phone).

Add your own questions and drag them into the order you want them answered. Supported field types:

  • Text and number, for things like a tax or company registration number
  • Dropdown, for a single choice from a list, such as annual order volume
  • Multi-select, when an applicant can pick several options, such as which product categories they stock
  • Radio, for a single visible choice, such as retailer or distributor
  • Checkbox, for agreements and opt-ins

Each field can be marked required, and every answer is stored against the application so you can see it when reviewing.

Reviewing applications

Open Registrations to see everything that has come in, filtered by pending, approved, and rejected.

  • Approve creates or updates the Shopify customer, applies the group's tag, and sends a branded approval email.
  • Reject sends a branded rejection email. No customer record is created.

Both emails carry your store name and branding rather than looking like a generic app notification.

Spam protection

The form uses a hidden honeypot field plus per-email rate limiting. Automated submissions are silently routed aside, and a repeat submission from the same address inside 24 hours is blocked. Genuine applicants are never shown a puzzle or a CAPTCHA.
